PATIENT INFORMATION

Insurance Cards and Photo Identification

When arriving to your appointment you will be asked to present your insurance card and photo identification at check-in.  Our staff will scan your cards and Id to ensure the correct information is on file.  Insurance coverage will be verified upon each visit.  If your insurance is not active you will be asked to pay for the visit at the time of service.

Co-payments and Deductibles

Payment for services are collected at check in.  Co-payments and deductibles will be collected on the day of service.  For your convenience we accept Visa, Mastercard, American Express, Cash or Check.

New Patients

We are happy to accept new patients to our practice.  After calling to set up your appointment you may print the New Patient Forms, fill them out ensuring past medical history is completed thoroughly and bring them in to your visit.  You should arrive 20 minutes in advance of your appointment to allow time for our staff to set up your account and chart.  Please remember to bring your Photo ID and Insurance card to your visit.

Appointment Cancellations

We ask that you call us 24 hours in advance to cancel your appointment to avoid a $25 cancellation fee.

Referrals and Authorizations

Please allow our office 72 hours to process referrals or prior-authorizations required by your insurance company. Patients should contact their insurance company to verify if a referral is necessary for specialist visits.  Please inform our staff if you will need a referral for an upcoming appointment.

Medication Refills

Medications are refilled within 72 hours of your request.  Please call our office 1 week in advance of running out of your medication.  If you have not been seen in our office within the last six months of your refill we will require that you make an appointment and see the doctor.  You may also request a refill on line through our secure patient portal.
